Client Bill of Rights
InTouch Medical Supply Client Bill of Rights is designed to in keep you Informed, InTouch and in Control of your health by protecting, and promoting your exercise of these rights. We respect and recognize your individual right for personalized healthcare for you, your caretakers and your property to be treated with the highest regards and consideration. We will provide you with the Bill of Rights at any time, upon request in a language and manner you can understand. You have the right to:
- Be fully informed in advance of initiating your care both orally and in writing of the care being provided, including:
- Changes to your care and supplies
- Payment for your care and service expected from third party payors
- Any and all charges for which you may be responsible
- Participate in the development and periodic revision of the plan of care; and ability to take part in deciding your plan of care and making modifications based on your needs and wishes
- Receive information about the scope of services that the organization will provide and specific limitations on those services
- Refuse care or treatment after the consequences of refusing care or treatment are fully presented
- Be informed of client/patient rights under state law to formulate an Advanced Directive, if applicable
- Have one’s property and person treated with respect, consideration and recognition of client/patient dignity and individuality
- Be able to identify visiting personnel members through proper identification
- Receive appropriate care without discrimination in accordance with physician’s orders
- Be free from mistreatment, neglect or verbal, mental, sexual, and physical abuse, including injuries of unknown source, and misappropriation of client/patient property
- Voice grievances/complaints regarding treatment or care or lack of respect of property, or recommend changes in policy, personnel or care/service without restraint, interference, coercion, discrimination or reprisal
- Have grievances/complaints regarding treatment or care that is (or fails to be) furnished or lack of respect for property investigated
- Confidentiality and privacy of all information contained in the client/patient record and of Protected Health Information
- Be advised on the agency’s policies and procedures regarding the disclosure of clinical records. If the client/patient has been adjudged incompetent under state law by a court of proper jurisdiction, the rights of the client/patient are exercised by whomever you appointed to act on your behalf. If a state court has not adjudged a client/ patient incompetent, any legal representative designated by the client/patient in accordance with state law may exercise the client/patient’s rights to the extent allowed by law
- Choose a healthcare provider, including an attending physician, if applicable
- Be informed of any financial benefits when referred to an organization
- Be fully informed of one’s responsibilities
